- Between 30-40% of health resources is wasted and common areas of inefficiency include, Human Resources for Health, commodity procurement and supply management and health system organization. Demand side strategic purchasing which ties spending to purchase of needed services that are most effective has been recognized as potent tool for enhancing efficiency of demand-side financing. However, the bulk of government spending in Nigeria is channel through supply-side as the health insurance penetration is still very low. Therefore, the need to prioritize supply-side strategic purchasing cannot be overemphasized.
- Adjusting Public Financial Management (PFM) structure by migrating to Program Based Budgeting which allows linkages between health funding and health outcomes is a promising approach for allocating resources to program areas that will bring highest level of impact and provide opportunity for efficient budget execution and revolutionize budget tracking.
- While World Bank and other partners have provided support that enables some states to develop administrative code as part of advancement towards International Public Sector Accounting Standard (IPSAS), states are yet to develop program code that will enable Program Based Budgeting and where program code exist it’s not used as the basis for budget, allocation, execution and monitoring. Another related PFM initiative in the horizon is Integrated Financial Management System (IFMS) which among other things, will ensure migration from paper based to fully automated financial accounting and reporting.

- Onehealth tool is a software application developed to support countries to estimate the resource requirements for a comprehensive and integrated national health strategic plan. The tool allows users to project health sector needs for years into the future, plan budgets, plan for numbers needed of specific health care providers, plan the number of units and costs of pharmaceuticals that will be needed, identify where bottlenecks are likely to occur in the projection and planning, interventions to apply to problems, and various other mapping tools for any selected country. It is thus primarily intended to inform sector-wide national strategic health plans and policies.
- STATA is a statistical tool of analysis which is critical in completing Health Account Studies. After the training, participants would be able to support and sustain tracking of core health system resources along the continuum of Anambra State SHDP II including the conduct of annual health account studies.

Rationale
- The current effort of the FMOH in coordinating health systems reforms is an effort that needs commendation. The National Health Accounts 2017 still shows low government spending/expenditure in health at all levels.
- To address limitations of funding to the health sector in Nigeria, there is a need to further analyze the resource needs estimates and financing plans for health priorities in line with the National Strategic Health Development Plan (NSHDP II) and use that analysis to identify the largest challenges and inform strategies for closing fiscal gaps. - hese strategies are intended to be shared with domestic and external sources of funding and help ensure that the FMOH is adequately funded through 2030 to ensure that set targets are achieved.
Scope of work
- This consultant will determine and revise original resource needs estimates of the NSHDP II and key health financing estimates at the Federal Level, review and update alternative projections and scenarios, review and update the estimate of the overall funding gap, and analyze the scope for increasing domestic financing for health in FMOH.
Specific Tasks
Undertake a desk review/literature review of relevant documents and local research and conduct an analysis of the current situation in FMOH:
- Enrich the desk review with stakeholder analysis, qualitative and quantitative data collection using developed tools engaging with the FMOH, NHIS, NPHCDA, other MDAs (Finance, Budget & National Planning, Justice, Labour, etc), CSO, private sector, etc.
- Conduct fiscal space analysis by revising original resource needs estimates of the NSHDP II and key health financing estimates at the Federal level, revising and, updating alternative projections and scenarios; revising and updating estimates of the overall funding gap, and analyzing the scope for increasing domestic financing for health in FMOH.
- Produce a report of the Fiscal Space analysis in FMOH
- Share and validate the fiscal space analysis report with stakeholders in FMOH
